Request for Notification of “My Number” (Individual/Corporate Number)

In accordance with the “Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures (My Number Act)” and the “Act on Submission of Statement of Overseas Wire Transfers for Purpose of Securing Proper Domestic Taxation,” the customer is required to notify his/her “My Number” (Individual Number / Corporate Number) to the financial institution conducting the transaction.
Individual Number is a 12 digit number which is assigned to every person possessing a resident record in Japan. My Number is assigned to foreign nationals as well if he/she has a resident record in Japan. Corporate Number is a 13 digit number which is assigned to “established registered corporations” such as kabushiki kaisha or other entities.
