Provision of Personal Information to Third Parties (SBI Remit NEOBANK Members Only) (SBI Remit/SBI Sumishin Net Bank / a supervising organization and/or a registered support organization, etc.)

To: SBI Remit Co., Ltd.

I hereby agree that SBI Remit Co., Ltd. (“SBI Remit”) may provide my personal information to SBI Sumishin Net Bank, Ltd. (“SBI Sumishin Net Bank”) or a supervising organization, etc. (referring to a supervising organization or a registered support organization that supports foreign technical intern trainees or foreign specified skilled workers; the same shall apply hereinafter) and other organizations as follows:

1. Provision of Personal Information to SBI Sumishin Net Bank

(1) Third party to which personal information is provided:

(i) SBI Sumishin Net Bank

(2) Purpose of use by the third party to which personal information is provided:

(i) To open a deposit account at SBI Sumishin Net Bank (SBI Remit Branch);

(ii) To provide information on SBI Remit NEOBANK, to manage the SBI Sumishin Net Bank (SBI Remit Branch) account after it is opened, and to respond to customer inquiries;

(iii) To exercise rights and fulfill obligations stipulated in contracts with customers or by law, etc.;

(iv) To cancel or restrict various types of transactions, and to manage issues arising after transactions are terminated; and

(v) For other purposes incidental to the above items.

(3) Information to be provided to the third party:

(i) Name, gender, and date of birth;

(ii) Postal code and address;

(iii) Term of residence in Japan;

(iv) Telephone number and email address;

(v) Customer code;

(vi) Occupation;

(vii) Company which the customer works for and supervising organization etc. which the customer belongs to;

(viii) Status of customer’s use of SBI Remit’s services; and

(ix) Information obtained by SBI Remit in the course of verifying identity and characteristics, etc.

2. Provision of Personal Information to the Supervising Organization and Registered Support Organizations, etc.

(1) Third parties to which personal information is provided:

(i) The supervising organization, etc. which the customer belongs to;

(ii) The organizations (including companies and associations) that are entrusted by the supervising organization, etc. which the customer belongs to and that are engaged in services for accepting foreign technical intern trainees or foreign specified skilled workers, such as livelihood guidance and livelihood support services etc., for the customer;

(iii) The organizations (including companies and associations) that are engaged in services for receiving foreign technical intern trainees or foreign specified skilled workers, such as livelihood guidance and livelihood support services etc., jointly with the supervising organization, etc. which the customer belongs to; and

(iv) The company which the customer works for.

(2) Purpose of use by the third parties to which the personal information is provided:

(i) To assist the customer in submitting payroll accounts to the company which the customer works for; and

(ii) For other purposes incidental to the above item.

(3) Information to be provided to third parties:

(i) Name;

(ii) Date of birth; and

(iii) The following information regarding the deposit account opened by the customer with SBI Sumishin Net Bank (SBI Remit Branch): Date of account opening, name of bank, name of branch, type of account, account number, and name of account.
